Mechanism of Action

The mechanism of action of glimepiride is to stimulate the release of insulin from the pancreas. Insulin is a hormone that helps the body use glucose for energy. In people with type 2 diabetes, the pancreas does not produce enough insulin or the body's cells do not respond to insulin as well as they should. This leads to high blood sugar levels.

Glimepiride works by binding to ATP-sensitive potassium channels (KATP) on the surface of pancreatic beta cells. This causes the channels to close, which leads to a depolarization of the beta cells. Depolarization triggers the release of insulin from the beta cells.

In addition to stimulating insulin release, glimepiride may also have some extrapancreatic effects. These effects may include increasing the sensitivity of muscle and fat cells to insulin and decreasing the production of glucose by the liver.

Special Precautions

Increased risk of CV mortality. Elderly; hepatic and renal impairment. Syndrome of inappropriate secretion of antidiuretic hormone (SIADH) in patients with CHF or hepatic cirrhosis. Monitor blood-glucose concentration. Pregnancy, lactation.
